Fill out and submit this form to the Patton Village Office of Emergency Management if you will require special assistance in the event of an evacuation or other emergency.  Your information is protected and kept in a private database in the Office of Emergency Management at Patton Village.  Once submitted, a member of the Patton Village Police Department will contact you to confirm your submission and check for any additional information.  All IP addresses are logged on each submission.
